Getting started is easy

Whether you’re new to the NDIS or changing providers, we’ll walk you through every step. There’s no cost to have a chat.

What happens, step by step

    Get in touch

    Call us on (02) 8080 4685, send a message or make a referral. We’ll call you back within 24 hours to have a friendly chat about what you’re looking for.

    Meet and plan

    We meet at your home, our office or wherever you’re comfortable. Bring family, a friend or your coordinator. We talk about your goals, your routine and your NDIS plan.

  3. 3

    Agree your service

    We put everything in a clear service agreement: what support you’ll get, when, and how much it costs. Nothing starts until you’re happy with it.

  4. 4

    Meet your team

    We introduce you to support workers who suit your interests, language and personality, before your first shift.

  5. 5

    Start, and keep checking in

    Your support begins. We check in regularly and review your support whenever your needs or goals change.

Paying for support

We work with every way an NDIS plan can be managed. Not sure which one you have? It’s written in your plan, or we can help you check.

NDIA-managed

The NDIA pays us directly from your plan.

Plan-managed

Your plan manager pays our invoices for you. We send them straight to your plan manager.

Self-managed

You pay our invoices and claim from the NDIA. We keep invoices clear and simple.

You can apply through the NDIS at ndis.gov.au or by calling 1800 800 110. We’re happy to point you in the right direction.

Frequently asked questions

Does the first meeting cost anything?

No. Our first meeting is free and there’s no obligation to go ahead.

Can I switch to Horizon Care from another provider?

Yes. Check the notice period in your current service agreement, then we’ll help make the change smooth.

Which areas do you cover?

We support people across Greater Sydney. If you’re just outside, ask us anyway.

Can I get support in my language?

Our team speaks English, Cantonese, Spanish, Arabic, Vietnamese, Malay and Indonesian. We can also arrange a free interpreter through TIS National (131 450).

What are your hours?

New enquiries: Monday to Saturday, 9am to 5pm. Current participants have access to 24-hour support.
